Hurst T Top mod decal

Does anyone know if the decal that hurst placed on the door from stating the modification to the car is available anywhere?  I am doing a complete restoration and I want to replace it.

i have found that npd, perfomance years  and year one also ames have them listed in their catalogs but i make all my decals anyway

True...they do sell them but they can be made yourself pretty easily also

In my restoration project - I purchased the Altered By Hurst label; and it has two areas for info.  Does someone have a photo or measurements whereas shows the correct location of this decal?  Also what was hand written or typed into the blank areas of the label?
